Understand vitamin B12 deficiency, possible causes, test results, and clinician-directed care.
Vitamin B12 is needed to make blood cells and keep nerves working. Deficiency can cause anemia, numbness, balance problems, cognitive changes, a sore tongue, or fatigue, and it is sometimes found before symptoms appear. Nerve damage can happen without obvious anemia, so real symptoms deserve prompt treatment.
The long-term plan depends on why B12 is low. A vegan diet without reliable supplementation may need oral replacement plus a permanent food or supplement routine. Pernicious anemia, stomach or intestinal surgery, celiac or Crohn’s disease, and other absorption problems may need high-dose oral therapy or injections for life. Metformin and acid-suppressing medicines can also contribute.
What to do
- Confirm the diagnosis in context. Serum B12 is useful but imperfect. Methylmalonic acid can help with a borderline result, though kidney dysfunction affects it. Check the blood count and folate too.
- Treat promptly when neurologic signs are present. If numbness, weakness, balance, cognition, or vision is changing, don’t wait months on diet alone.
- Choose a route that fits the cause. High-dose oral B12 works for many people because a small amount is absorbed passively. Injections may be preferred for severe symptoms, very low levels, uncertain adherence, or significant malabsorption.
- Use dependable food sources. Meat, fish, eggs, dairy, and fortified foods provide B12; unfortified plant foods don’t reliably.
- Review your medicines. Metformin and proton-pump inhibitors can matter clinically. Don’t stop them on your own; decide whether monitoring or replacement solves the problem.
- Find the reason for recurrence. Ask about pernicious anemia, prior stomach or bowel surgery, autoimmune disease, celiac disease, inflammatory bowel disease, and restrictive eating.
- Plan maintenance. Correcting the level once is not enough if the cause remains. A lifelong replacement schedule can be simple and effective.
A simple plan
Write down baseline B12, methylmalonic acid if measured, blood count, neurologic symptoms, diet, medicines, surgeries, digestive conditions, and the suspected cause. Agree on oral or injected replacement and when to follow up.
For six to eight weeks, take the prescribed replacement every time. If diet is the cause, settle on a permanent daily or weekly supplement or reliable fortified foods. Once a week, not constantly, note numbness, balance, energy, cognition, and mouth symptoms.
Repeat the labs or clinical assessment at the planned time. A high serum B12 after supplementation is not a reason to stop maintenance; read it against the dose and your clinical response.
How to know it is working
What to expect
Blood-cell production can pick up within days to weeks; nerve recovery may take months. Serum B12 rises quickly after treatment, and that alone does not prove tissue recovery. The cause decides whether therapy can stop or must continue indefinitely.
Don’t compare your maintenance dose with someone else’s without comparing causes. Filling a dietary gap may take a modest, dependable oral routine; pernicious anemia or major malabsorption may need lifelong high-dose oral therapy or injections. What matters is whether your route and schedule reliably prevent recurrence. If you get injections, book the next dose before leaving the appointment, so a scheduling gap doesn’t become a relapse.
